Ordering Repeat Prescriptions

Vision Online Registration Form - Please fully complete this form and return to Reception. This must be handed in with a valid form of identification e.g. photo I.D.  or Passport. You will then be sent a letter with details and a secure password to access online services. If you experience problems with the system please contact the Practice. Patients must be ages 16 and over. 

Please note, when you are setting your account up, ensure you have a different user name than your email address as the system will not send you a confirmation code which you will require to complete your online registration.

Alternatively, patients can phone our prescription voicemail system on (01698) 456622 (available 8am-6pm Monday to Friday (excluding public holidays). All you have to do is leave your name, date of birth, the medication you require and whether you wish to collect at the surgery or send the medication to a local pharmacy.

Please allow 4 full working days for prescriptions to be processed and remember to take weekends and bank holidays into account.

We do not issue long-term prescriptions for Diazepam.

Medication Reviews

Patients on repeat medication may be asked to see a GP Practice Nurse or Practice Pharmacist to review regular medications. A notification should appear on your repeat slip when you are due a review. 

Co-codamol Shortage

There is a temporary shortage of Co-codamol 30mg/500mg tablets. Your GP or Pharmacist may not be able to give you your usual tablets during this time. Please make an appointment with one of our practice Pharmacists to discuss your medication if you have any concerns. For further information please click here.
